the rest seems fine. As Karl-Henry mentioned lookups are worth caching.
Probably it's hard to change logic not to use large lists if they are deemed necessary, but the larger they are the more they slow down the process.
Is 150K docs the upper limit? There are already a lot of docs in there? At that rate ther must be many. Millions? Do many get purged as well? Meaning db full of deletion stubs? Is it replicated or you can delete without leaving stubs?
Could there be some views in db that try to get updated while you keep creating them thus slowing down the server?
